Forms

New Student Form is required by the OSSAA and must be completed by every new student at Bishop McGuinness.

Physical Form is required every school year to participate in any OSSAA sport.  Students cannot participate in practice or games until this form is completed.

Emergancy Medical Information Form must be completed for every student athlete. 

Transportation and Equipment Form must be completed for every student athlete.

The Concussion Fact Sheet must be read by every student athlete and parent / legal guardian of a student who participates in sports.  The Concussion and Head Injury Acknowledgement Form must be completed and signed by both the student athlete and parent /legal guardian before the student can participate in practice or games.
