I, the undersigned, am the parent or legal guardian of the child listed abouve. I hereby give my permission for my child to attend the school dance event scheduled to be held on October 23rd, 2026 at Bishop McGuinness Catholic High School.

I acknowledge that participating in the school dance involves certain inherent risks, including but not limited to, the risk of personal injury, property damage, or loss. I voluntarily assume all such risks and agree to release and hold harmless Bishop McGuinness Catholic High School, its employees, volunteers, agents, and any affiliated entities, from any and all claims, liabilities, or causes of action arising out of or related to my child’s participation in the dance event.

In the event of an emergency, I authorize Bishop McGuinness Catholic High School and its representatives to take any necessary action, including medical treatment, to ensure the safety and well-being of my child. I understand that I will be notified as soon as reasonably possible if such an emergency occurs.

I have read and understood this waiver and liability release statement, and I voluntarily agree to its terms.
